Measure any other necklace you own. Or take a piece of string and put it over the neck. Then measure the string.

Your measurement most likely will be somewhere between sizes. Than if you prefer your necklace sitting tighter opt for a smaller size (you still will have 5cm (2 inches) extender). If you prefer your choker be looser, than choose bigger size.
